Membership Discontinuation Form - When a member depledges, deactivates, transfers, or goes Alumni status, you can use the following form to notify our office. By completing this form, that member will officially be removed from your roster.
Officer Update Form - If you have an officer that changes at any point in the year, please use this form to update our records. You do not need to use this form after formal elections—only if there is a change during the semester, DO NOT use this for your major election process.

Fraternity Affiliation Agreement and Grade Release
The following form MUST be completed within 72 hours of a new member accepting a bid. This will place new members on your roster with the office and allow you access to their GPAs

Continuous Open Bidding Acceptance Agreement and Grade Release
The following form MUST be completed within 24 hours of a new member accepting their bid. (Applying to COB only) This will place new members on your roster with the office and allow you access to their GPAs.

Event with Alcohol Registration Form
Events meeting the following criteria must register with IFC/Panhel and abide by guidelines stated in this policy:
- Events held in Chapter Houses – if number of non-member guests exceeds or equals 50% of Chapter membership and alcohol is present. (IFC)
- Events held off-campus – Alcohol is present, non-members are present as guests, and the event is hosted/co-sponsored off campus. (IFC & PHC)
- Overnight events off-campus – an off-campus event that does not include alcohol must include the Student Travel Policy. (IFC & PHC)
You must turn in the Event Registration Form a minimum of five (5) days in advance of event.
