Enrollment Health Form Deadlines
All new students must complete the Enrollment Health Forms through the Wellness Center patient portal before arriving on campus.
Student-athletes: You must also complete separate Athletic Health Forms in the Athletics Portal in addition to the forms listed below.
Please allow 7–10 days for staff to review and verify your forms.
How to Complete Your Enrollment Health Forms
Step 1: Log In
Visit the student health portal:
https://westminster.medicatconnect.com
Step 2: Complete Required Forms
In the portal menu, select “Forms.”
Fill out and submit all forms with a red asterisk (these are required).
You will see the following forms:

Health History
-
Tuberculosis (TB) Screening Questionnaire

If you answer “Yes” to any question, a TB test is required (usually done during your physical exam).

If you answer “No” to all questions, no TB test is required.

Meningococcal Acknowledgment and Waiver
Forms in the Disability section only need to be completed if they apply to you.
Step 3: Enter Immunization Dates
In the menu, select “Immunizations.”

Use the blue drop-down boxes to enter the dates of your vaccines.

Click Submit when finished.
Step 4: Upload Your Immunization Record
In the menu, select “Uploads.”

Next to “Immunization Record,” click Select File and upload your official record.

Upload any additional health documents you wish to share.
This upload is required so staff can verify the immunization dates you entered.
Step 5: Enter Emergency Contacts
In the menu, select “My Profile.”

Enter your emergency contact(s).

Optional: You may also share your gender identity, pronouns, and sexual orientation.
Where to Get Your Immunization Records
You can obtain your immunization records from:
State registry contact information is available on the CDC website (search “CDC immunization registry contacts”).
